Registering as a patient
In order to register with the practice, patients must live within the practice boundary (contact
reception on 0121 429 1572 to enquire whether your location is covered).
Please note that we are unable to register patients residing outside of this area.
You will be asked to complete a New Patient Registration form and to attend an NHS Medical Health-check (click here for details) with the practice nurse, in order to discuss your medical needs and medication requirements. Once we apply to the health authority for your full medical records, it may take up to three weeks for them to be transferred across from your previous GP practice.
